Contact details of the complaining party such as name, address, email, phone number, and fax number;
A statement confirming unauthorized use of the material;
An affirmation of the accuracy of the information provided and the authority to act on behalf of the copyright holder.

Counter Notification

If you wish to dispute a takedown notice, you can send a counter notification to our DMCA Agent. This notification must include:

Your signature;
Description of the removed material and its original location;
An affidavit stating the removal was due to error or misidentification;
Your contact details and consent to the jurisdiction of the court.

Policy on Repeat Infringement
